Rewards & Incentives
Welcome to Gold Kidney’s reward program
The Rewards and Incentives Program is designed to allow all members to earn reward credits in the amount of $25 for completing wellness activities for healthy behaviors.*
A total of twelve (12) wellness activities can be completed per year based on your health, recommended screenings, or existing health conditions, earning up to a maximum of $300 per year. Rewards do not roll over to the following year.
*You can earn up to $300 maximum per year, annually.
WELLNESS ACTIVITIES INCLUDE
- Health Risk Assessment within 90 days of enrollment
- Annual wellness visit
- Flu vaccine or covid vaccine or booster
- Fall Risk or Bladder Control Risk assessment
- Participation in Gold Kidney quality survey
- Participation in PCP visit survey
- Preventive cancer screenings
- Diabetic retinal eye exam
- Hemoglobin A1c exam, two times per year
- Controlling Blood Pressure exam, two times per year
- PCP visit within seven days of emergency room visit
- Medication reconciliation visit with PCP within 14 days of inpatient discharge
