Your Alivi Gold Kidney Prepaid Visa® Card is loaded on a quarterly basis with an allowance that varies according to your enrolled plan. Member chooses what service to purchase, which vendor to use, and how much to spend on each service to the quarterly benefit limit associated with your particular plan.
Alivi Gold Kidney Prepaid Visa® Card is issued by Avidia Bank, pursuant to a license from Visa U.S.A. Inc. Valid only in the U.S. No cash access. Other languages are available upon request.
Dental, Hearing & Vision
Use your Alivi Gold Kidney Prepaid Visa® Card allowance to pay for dental exams and procedures, hearing check-ups and hearing aids, or eye exams and glasses. Choose the provider of your choice and pay for it on your Flex Card. No insurance claims to submit.
Allowances don’t roll over from quarter to quarter.

- A fuel allowance that can be used at any gas station that accepts Visa
