Gold Kidney offers a Flex Card that works just like a Mastercard. Your Flex Card is loaded on a quarterly basis with an allowance that varies according to your enrolled plan. Member chooses what service to purchase, which vendor to use, and how much to spend on each service to the quarterly benefit limit associated with your particular plan.
Dental, Hearing & Vision
Use your Flex Card allowance to pay for dental exams and procedures, hearing check-ups and hearing aids, or eye exams and glasses. Choose the provider of your choice and pay for it on your Flex Card. No insurance claims to submit.

Rewards & Incentives
Earn additional allowance dollars by visiting a provider for a preventive service and receive a reward of up to $25 or maximum of $300 on your Flex Card per service. Eligible services include:
- Annual Wellness Visit
- Annual Flu Vaccine or COVID vaccine or booster
- Mammogram, Prostrate Exam, Cervical or Prostate Cancer Screen
- HbA1C readings two times per year
- Completing a Health Risk Assessment Participating in Gold Kidney Quality Survey
- Medication reconciliation within 14 days of an inpatient
- And other services

Special Supplemental Benefits for the Chronically Ill
If you have one of 16 chronic conditions and participate in case management, you may be eligible for:
- A monthly healthy food allowance on your Flex Card which can be used at Walmart, CVS, Target, Winn Dixie, Walgreens, Publix, Kroger and more
- A fuel allowance that can be used at any gas station that accepts Mastercard

Over the Counter Medications & Protein Supplements
Quarterly allowance for OTC medications and protein supplements.
- Buy in store at Walmart, Target, CVS, Walgreens
- Or order online from Alivi Health Catalog and have it delivered to your door
