Special Supplemental Benefits for the Chronically Ill (SSBCI)
Special Supplemental Benefits for the Chronically Ill (SSBCI) are benefits that can be offered to Medicare Advantage members with one or more complex chronic conditions, who are at high risk for hospitalization or adverse health outcomes, and who require intensive care coordination.
Monthly allowances for healthy food and fuel benefits are a part of SSBCI provided to Gold Kidney members. ALL the following eligibility guidelines must be met and participation in case management activities is required to receive these benefits.
To qualify you must:
have at least one qualifying chronic condition (see below) -and-
be at high risk for hospitalization or other adverse health outcome -and-
require intensive care coordination.
Those who qualify must also participate in case management.
To qualify you must be assessed by a case manager to determine if you meet the chronically ill criteria. Not everyone will qualify.

The benefits mentioned are part of a special supplemental program for the chronically ill. Not all members qualify.
