Your Alivi Gold Kidney Prepaid Visa® Card is loaded with allowances that vary according to your enrolled plan. Member chooses what service to purchase, which vendor to use, and how much to spend on each service to the monthly and quarterly benefit limits associated with your particular plan.

Alivi Gold Kidney Prepaid Visa® Card is issued by Avidia Bank, pursuant to a license from Visa U.S.A. Inc. Card is serviced by Fidelity Information Services, LLC (FIS).

Dental, Hearing & Vision

Use your Alivi Gold Kidney Prepaid Visa® Card allowance to pay for dental exams and procedures, hearing check-ups and hearing aids, or eye exams and glasses.  Choose the provider of your choice and pay for it on your Flex Card.  No insurance claims to submit.

Allowances do rollover from quarter to quarter for 2025 plans.

  • A fuel allowance that can be used at any gas station that accepts Visa
